Dr. McCutcheon made the trip to Russellville with her own baby to have surgery with Dr. Dew. Brianna was suffering from bilateral luxating patellas…

PATIENT SUMMARY:
Corrie is a Pomeranian. He is 3.5yrs old and weighs 4.1#. His referring veterinarian is Dr. Steven Ambrose of Cloverleaf Animal Clinic. Corrie has been using the right fore well since the last exam confirmed fracture healing. He has had 3 cycles of antibiotics to treat a cyst like structure on the same just off the incisional scar. On radiographs there is no evidence of infection at the bone level. Dr. Dew discussed low grade infection, cyst and tumor. He suggested plate removal, culture and cyst excision with histologic assessment. His owner wished to proceed with the diagnostic and surgical plan, and utilized transport to Russellville.
TREATMENT:
Dr. Dew used a craniolateral approach for Corrie’s surgery. There was no evidence of osteomyelitis or obvious extension of abnormal tissue deep to dermis. He performed an elliptical excision of cyst 1mm margins. This was submitted to AR-State lab, removed plate, culture screw-AR State lab. Corrie’s prognosis is good for soft tissue healing. The pathology and culture results will provide valuable information concerning further treatment and prognosis.

PATIENT SUMMARY:
Maddie is a Labrador Retriever. She is 8.5 yrs old and weighs 92#. Her referring veterinarian is Dr. Jon Southerland with Kiehl Avenue Animal Clinic. She presented to Dr. Southerland with Bilateral rear limb lameness, and he identified Bilateral CCL rupture. Upon examination by Dr. Dew, there was positive anterior drawer and medial buttress consistent with Bilateral CCL rupture. He discussed TTA (tibial tuberosity advancement) and EFS (external fascial strip) procedures with Maddie’s owner, and she decided to proceed with Bilateral TTA surgeries.

TREATMENT:
Dr. Dew used a medial approach with medial arthrotomy. Kyon hardware used was as follows: 12x22mm basket, 6 hole plate & fork, and 4 titanium cortical screws. A cancellous bone graft was placed to promote bone healing. Maddie’s prognosis is good for healing, improved function and decreased progression of degenerative joint disease (arthritis). Nursing care, physical therapy, exercise restriction and the use of Glycoflex will play critical roles in an uncomplicated recovery and optimization of long term function. With the TTA procedure, it is not uncommon to see some bruising and edema around the hock (ankle) and on the inside of the leg below the incision. These changes are not painful and will resolve without treatment in 3-10 days. With having both legs corrected at once, “Maddie” will benefit from the use of the Help ‘Em Up Harness when rising and walking for the next 1-14 days.

PATIENT SUMMARY:
Boogie is an Exotic Geoffroys Cat. She is 5 months old and weighs 3#. Her referring veterinarian was Dr. Teresa Durham from All For Pets. Boogie jumped from her owner’s head, and is now acutely non-weight bearing lame on left forelimb. Dr. Durham identified a comminuted fracture of the distal humerus. Dr. Dew discussed internal pin/wire fixation, and gave a good prognosis for healing, with the need for activity restrictions. Boogie’s owner wished to proceed with surgery.
TREATMENT:
Dr. Dew used a lateral approach with olecranon osteotomy. The small comminutions did not allow for a complete reduction; obliquity was short allowing only single cerclage. There was good ROM post stabilization. The fracture was stabilized using divergent 0.045 wires and a 20 gauge cerclage. The olecranon was stabilized with 0.045 IM wire and periosteal/fascial sutures. Boogie’s prognosis is good for normal osseous healing. Exercise restrictions must be followed to maximize the prognosis for uncomplicated healing.

Lindsey is a Terrier Mix weighing 40# and is 2yrs old. Her referring veterinarian is Dr. J.K. Montgomery of Fox Ridge Animal Hospital. Lindsey had Bilateral Tibial Tuberosity Transpositions (TTT) surgeries 1 year ago. She returned to our facility to have her Left cruciate corrected by External Fascial Strip – EFS.

TREATMENT:
Dr. Dew used a lateral approach with lateral arthrotomy. He explored, debrided, lavaged, and imbricated joint EFS using medial buttress suture #0-PDS. Lindsey’s prognosis is good for healing, improved function and decreased progression of degenerative joint disease (arthritis). Nursing care, physical therapy, exercise restriction and the use of Glycoflex will play critical roles in an uncomplicated recovery and optimization of long term function.

PATIENT SUMMARY:
Lola is a Boxer, and she weighs 70.9# and is 4yr and 5mo old. Dr. Paul Seminara of Green Mountain Animal Hospital is her referring veterinarian. Lola came to us in December of 2009 for a Left TTA (tibial tuberosity advancement), and had been doing very well on it. She had now been diagnosed with a Right CCL (cranial cruciate ligament).

TREATMENT:
Dr. Dew used a medial approach with medial arthrotomy. A cancellous graft was harvested from distal femur. Kyon implants used for the TTA surgery were: 9X19mm basket, 5 hole plate & fork, and 4 titanium screws. Lola’s prognosis is good for healing, improved function and decreased progression of degenerative joint disease (arthritis). Nursing care, physical therapy, exercise restriction and the use of Glycoflex will play critical roles in an uncomplicated recovery and optimization of long term function.
